Please plan to arrive 15 minutes before your scheduled appointment to allow yourself enough time to complete our entire registration process.
It is our policy to collect fees or co-payments at the time of your visit unless you make other arrangements. As a courtesy, we will bill your insurance carrier. Please bring a picture identification, a copy of your insurance card and a referral from your primary care physician if this is required by your insurance carrier.
