Your insurance coverage is a contract between you and your carrier. We will file appropriate reimbursement forms with your insurance carrier. You are responsible for payment of deductibles, co-payments and remaining balances at the time of your office visits. In some cases, you may be required to make payment prior to your insurance carrier making payment.

If your insurance carrier requires a referral or prior authorization from your primary care physician, please bring that informationwith you to your appointment. If your insurance carrier fails to pay us within 90 days, you will be responsible for payment for our services.
